Money Matters Online Confidentiality and Consent Statement
Wellspring Cancer Support Foundation holds all personal information relating to our members in strict confidence and has a stringent confidentiality policy that Money Matters Case Workers must follow. We will not disclose any information without your prior consent, unless we are legally required to do so.
By agreeing to the terms of our confidentiality statement you agree that the Money Matters Case Worker is providing information and guidance to help you to the best of their ability. You agree that none of the Case Workers, Wellspring or any of its directors, officers, employees or volunteers is liable or responsible for any errors or omissions that might occur in connection with the information provided to you.
You authorize Wellspring to contact other agencies or organizations to obtain information to assist with your specifically requested needs. This will only be done following direct communication with you. By giving consent to the Money Matters case worker to advocate on your behalf, you understand that information about your cancer diagnosis may be communicated to those agencies or organizations.
You give permission to other agencies or organizations that you may be involved with to provide information to Wellspring relevant to the specifically requested need and/or to advocate on your behalf. This may only be done following direct communication with you.
