Cancer Transition Coaching
The Cancer Transition Coaching program is designed specifically for patients between 3 and 18 months post-treatment.
The goals of the Cancer Transition Coaching program are to:
- Help you successfully transition away from active treatment and follow-up care
- Address your concerns so that you feel heard, supported and encouraged, and
- Provide you with helpful information, resources and community-based programming that will aid in your individual wellness needs, plans and strategies
You will be matched with a specially-trained peer volunteer, with whom you will meet for six private sessions to talk about your cancer experience, ask questions and discuss any concerns, needs, circumstances, wishes and goals you might have related to your post-treatment experience.
All information is discussed in a supportive, confidential and non-judgmental manner, and your personal Cancer Transition Volunteer Coach will be here to help and support you every step.
Note: To be part of the Cancer Transition Coaching program, you must be between 6 and 18 months post-cancer treatment.
